Waiting is reasonable for mild acne and expensive for inflammatory acne, and the difference is scarring. Most teenagers do outgrow the condition, but the lesions that scar do their damage while everyone is waiting — and those scars are permanent, far harder to treat than the acne was, and something I see in patients who arrive in their late twenties wishing someone had intervened. At our Busan clinic the assessment decides which category a teenager is in, and where the answer is mild we say so and treat conservatively rather than starting anything unnecessary.

Minimally, and only where they add something prescribed treatment cannot. Gentle clearing and calming work has a place alongside medical management, and there is no reason a teenager cannot have an aqua peel. What we do not do is sell adolescents treatment programmes, laser courses or packages — teenage skin responds well to medical treatment, and adding procedures mainly adds cost. At our Busan clinic any procedure is discussed with the parent or guardian and included only when the doctor can explain what it contributes.

What does the consultation actually involve?

A parent or guardian attends throughout. The doctor asks what has been tried, reviews the daily routine — most teenagers arrive over-cleansing, and correcting that alone helps a proportion of them — examines the skin, grades severity and scarring risk, and explains the plan to both the teenager and the adult. Prescribed treatment is described with its duration and what to expect. Everything is stated in writing at our Busan clinic so that the plan is followed at home rather than half-remembered.
