The orbicularis oris circles the mouth and contracts constantly — speaking, eating, drinking, expressing. The skin above the lip is thin, has few oil glands, and is heavily sun-exposed. Over years that combination etches vertical lines, and volume loss along the lip border removes the support that kept the skin taut.
Smoking accelerates all three, which is why the lines carry that name, but non-smokers develop them from the same mechanism given enough time. Treatment addresses the causes present: micro-dose botox for muscular pursing from ₩20,000, fine filler or boosters for the border, and resurfacing for etched skin. Why is this area harder to treat than the rest of the face?
Because everything works against you at once. The muscle contracts constantly and cannot be significantly weakened without affecting speech, drinking and expression. The skin is among the thinnest on the face, so filler shows through easily and heals visibly. And the lip border is a defined anatomical line that patients notice immediately if it moves even slightly. So the margin between improvement and an obviously treated mouth is narrower here than anywhere. At our Busan clinic that means very small doses, a mandatory two-week review, and a willingness to accept a modest result rather than push for a dramatic one.

It can if dosed as though it were a forehead, which is exactly why it is not. The orbicularis oris is essential for speech, drinking through a straw, whistling and pronouncing certain sounds, so doses here are a fraction of those used elsewhere — enough to ease constant pursing tension, not enough to weaken function. Patients who have had difficulty elsewhere almost always received too much rather than the wrong treatment. At our Busan clinic the two-week review specifically checks speech and drinking, and any effect at these doses resolves within weeks rather than persisting.

Because the mechanism is not smoking, it is repetition plus thin sun-exposed skin. The muscle around your mouth contracts thousands of times a day whether or not a cigarette is involved, and the skin above the upper lip is thin, low in oil glands and constantly exposed. Smoking accelerates this considerably — through both the pursing action and the vascular damage — which is why the name stuck.
