Preventative botox is one of the few aesthetic claims with a plausible mechanism behind it, and one of the most overstated in how it is sold. This guide separates what the mechanism supports from what marketing has added, and gives a test you can do in a mirror before booking anything.

What preventative botox actually prevents — the Busan view

A line becomes permanent through repetition, as we explain at our Busan clinic. The muscle folds the skin in the same place thousands of times, and as collagen thins with age the fold stops springing back — a dynamic line, visible only in expression, becomes a static one visible at rest.

Reducing how forcefully that muscle folds the skin slows the rate at which the crease sets. That is the whole mechanism, and it is genuinely plausible. What it does not do is stop collagen loss, sun damage or volume change, which are the other half of facial ageing and arguably the larger half.

The honest claim is slowing etching, not preventing ageing. Daily sun protection almost certainly does more for a twenty-five-year-old's face over thirty years than botox does — and it is free.

The test for whether it is too early, before booking in Busan

Before your Busan consultation, stand in front of a mirror with your face completely relaxed. Look at your forehead, between your brows and at the outer corners of your eyes.

  • No lines visible at rest, none in expression — too early. There is nothing to slow yet.
  • No lines at rest, clear lines in expression — this is the window. Treatment slows the etching before it begins.
  • Faint lines still visible at rest — etching has started. Treatment now is both preventative and corrective.
  • Clear lines at rest — no longer preventative. Still worth treating, with skin work alongside.

The honest risks of starting early

  • Treating what was not a problem — the commonest issue, and it is financial rather than medical
  • Muscle habit change — some patients recruit neighbouring muscles differently over years of treatment
  • Expectation drift — starting young can normalise a smoother baseline than your face would otherwise have had
  • Dependence on maintenance — nothing is permanent, and lines return to their natural rate when treatment stops

The last point deserves clarity because it is frequently misstated in both directions. Stopping botox does not make lines worse than they would have been — it returns you to the trajectory you were on. What it does not do is preserve the benefit indefinitely, so the honest question before starting is whether you intend to maintain it for years.
