What is Sculptra, and what does it do?

Sculptra is poly-L-lactic acid (PLLA), a biodegradable polymer in microparticle form, suspended in water and placed deep in the face where volume has been lost. It adds nothing on the day; over the following weeks the particles trigger fibroblasts to lay down new collagen around them, and as the PLLA itself dissolves over months the collagen stays. The result is volume that the face grew rather than volume that was placed, which is why Sculptra patients look rested rather than filled, and why the effect lasts eighteen months to two years.

Downtime after Sculptra, and flying home

Mild. Swelling for one to two days from the water the product is suspended in, which is not the result and goes down; a possible bruise. Fly after 24 hours. The real change begins at six weeks.

How many Sculptra vials will I need?

One to two per session, two sessions six weeks apart, for most faces; one vial per session for mild deflation or the temples alone. Quotes vary wildly because some clinics count a course of four vials as "the treatment" and others quote one. Why does Sculptra take months to work, and can it be reversed?

Because the volume is collagen your own fibroblasts make around the particles, and that takes six weeks to start and three months to peak. It cannot be dissolved the way HA filler can, which is why the dose is conservative, the plane is deep, and the massage protocol matters. In practice, "going wrong" with Sculptra means a small nodule, which is treated with massage or a small injection and is uncommon in experienced hands.

What is the swelling on day one, if not the result?

Water. Each vial is reconstituted with sterile water and lidocaine, and that fluid is what makes the face look full on the day. It absorbs within a day or two, the face returns to how it was, and nothing visible happens until the collagen starts at six weeks. Patients who are not warned think it failed; patients who are warned wait.

How strict is the massage protocol really?

Strict for five days. Five minutes, five times a day, over every treated area, spreads the particles evenly and is the single most effective way to prevent nodules. We show you at the clinic and message a reminder. After five days the particles are fixed and massage is irrelevant.

Is Sculptra right for someone who has lost a lot of weight?

Often it is the best option, because weight loss deflates the whole face evenly and Sculptra rebuilds the whole face gradually. Patients on Mounjaro at our Seomyeon skin clinic are told at the first consultation that facial volume will follow the body, and that Sculptra or filler can be planned around it.
