Clinics describe almost everything as comfortable, which helps nobody decide. What follows is the ranking we would give a friend, including the treatments that are genuinely uncomfortable and the ones where numbing makes less difference than you would expect.

Which treatments need no numbing at all?

Most botox. When is an injected anaesthetic used?

For thread lifting and mole removal, where topical cream is inadequate. Threads need infiltrated local along each vector, which takes ten to fifteen minutes and makes the insertion itself largely painless. Mole removal needs a small injection at each lesion — the injection is the uncomfortable part and lasts seconds; the laser afterwards is felt as pressure and warmth.

What is a nerve block and when is it offered?

A local anaesthetic placed near a nerve trunk rather than at the treatment site, numbing everything downstream. It is used mainly for palm treatment in hyperhidrosis botox, where palms are among the most sensitive areas on the body and topical cream barely reaches. A dental block is also available for lip filler for patients who would rather feel nothing at all.

An honest ranking, most to least uncomfortable

Fractional resurfacing is the most uncomfortable treatment offered here, and numbing reduces rather than removes it. Ultherapy follows — brief deep heat along each line, strongest at the jawline, and topical cream does not reach that depth. Then palm and sole hyperhidrosis. Then under-eye and lip filler. Why does numbing not help with some treatments?

Because topical anaesthetic reaches only a few millimetres. Focused ultrasound deposits energy at 4.5mm at the SMAS layer — well below where cream penetrates — so the sensation is unchanged by it. The same applies to deep periosteal filler placement. Where numbing genuinely helps is anything shallow and repeated: boosters, microbotox, resurfacing, and the surface component of laser work.

Why do published pain descriptions differ so much?

Because pain is subjective and clinics have an interest in the lower estimate. The same Ultherapy session gets described as mildly warm on one website and genuinely difficult on a patient forum, and both are honest reports from different people. Anxiety, sleep, caffeine and where you are in a menstrual cycle all measurably shift pain threshold. The useful framing is not what a treatment feels like in the abstract but what it felt like to you last time — which is why your response is recorded in your notes and used to adjust the approach on a repeat visit.

Does a more comfortable device mean a weaker result?

Sometimes, and it is worth knowing which. Sofwave is more comfortable than focused ultrasound partly because it works at 1.5mm rather than 4.5mm — less depth means less sensation and also less structural lift. Thermage FLX is more comfortable than earlier generations because of better cooling and vibration, which costs nothing in effect. The first is a genuine trade-off, the second is engineering. Where a clinic offers a notably gentler version of something, asking which of the two it is takes one question.

What can be done if I am anxious about needles?

Say so at booking rather than on the table, and several things change. Numbing is applied regardless of whether the treatment normally needs it. You are treated lying down where that applies. Does numbing affect the result?
