Not if the dose and placement are right, and that is precisely why these are the most conservative injections on my menu. The depressor anguli oris sits directly beside the muscles that lift the mouth into a smile, so an excessive dose or a millimetre of drift can flatten one side of the smile for months. My rules at our Busan clinic are fixed: assess in motion rather than from a photograph, dose deliberately below what looks sufficient, and review at week two with the explicit option to add. Patients who ask for a stronger first dose are told why the answer is no.

The depressor anguli oris pulls each mouth corner downward, and in some faces it is strong enough that the resting mouth reads as displeasure to everyone except the person wearing it. Easing that pull with a small dose lets the corners sit level, which changes how strangers read your expression without changing how you express. The honest caveat: where volume loss around the mouth is doing the pulling, filler rather than botox is the answer, and the assessment at our Busan clinic separates them.
Why are these doses so much smaller than forehead botox?
Because the consequences of excess are different. Over-dosing a forehead makes brows feel heavy, which is unpleasant but reads as a rested face rather than a wrong one. Over-dosing around the mouth affects the smile, which is the most recognisable thing about a face and the change people notice immediately. The muscles here are also genuinely small — a few units achieve what dozens would elsewhere. At our Busan clinic these areas are dosed at a fraction of forehead levels, reviewed at week two, and topped up only if the mirror asks.
