Goal one — volume

Goal three — shape and asymmetry

Correcting an uneven lip, a thin upper lip against a full lower one, or a downturned corner. This is placement rather than quantity — small amounts positioned specifically, sometimes with a few units of botox at the corners where muscle is pulling the mouth down. Most faces are mildly asymmetric and complete symmetry is not the target; visible imbalance is.

Goal four — balance with the rest of the face

Lips read in proportion to the chin and the lower face, not in isolation. A recessed chin makes lips look protrusive; a heavy lower face makes them look small. Patients occasionally arrive for lip filler and leave with chin filler instead, because the proportion rather than the lips was the issue. Assessing in profile catches this — front-facing photographs do not.

Why does this area punish overtreatment fastest?

Because lips are small, mobile and centrally placed, so a proportional change that would be invisible in a cheek is obvious in a mouth. Half a cubic centimetre distributed across a midface is undetectable; the same volume in lips is a noticeable difference in shape. Add that lips are the part of the face people watch while you speak, and the margin between improved and overdone is narrower here than anywhere except the under-eye. Conservative first sessions are not caution for its own sake — they reflect where the tolerance actually sits.

How does this fit with the rest of the lower face?

Closely, and it is why lips are assessed alongside rather than alone. A softening jawline changes how the mouth sits, which lifting addresses. Perioral lines above the lip are skin quality rather than volume, and respond to boosters or resurfacing rather than to filler placed in the lip itself. And a downturned corner is frequently muscle. What is a gummy smile and what treats it?

Excessive upper gum shown on smiling, produced by a hyperactive levator muscle rather than by anything about the lips. Why do lips swell so much?
