Is thick skin actually a disadvantage?

Cosmetically it reads coarse, and that is a legitimate complaint. Biologically it is closer to an advantage — thicker skin wrinkles later, holds structure longer, and tolerates procedures better than thin skin does. Patients with thick sebaceous skin frequently reach their fifties with markedly fewer fine lines than thin-skinned contemporaries, which is worth knowing before spending on refinement. So at our Busan clinic I treat the texture that bothers you while being clear that what we are refining is also protecting you, and I keep resurfacing conservative rather than chasing a smoothness that would cost that advantage.

Does exfoliating harder help or make it worse?

Worse, reliably. Home exfoliation works on the outermost layer while the thickening you are seeing is structural — enlarged sebaceous units, disorganised collagen from sun, or defensively thickened tissue. Scrubbing that surface irritates it, and irritated skin in Asian skin types frequently responds with pigment, so patients often add a second problem to the first. At our Busan clinic the standing instruction is to stop aggressive exfoliation entirely and let controlled resurfacing do the structural work that scrubbing was never capable of.

Are the small bumps sebaceous hyperplasia or something else?

Sebaceous hyperplasia has a characteristic look — small, soft, yellowish, often with a central dimple, sitting on forehead, nose and cheeks in oily skin. But several other things look similar at a glance, including milia, small warts and occasionally lesions that need proper assessment, which is why every raised bump is examined rather than assumed. At our Busan clinic sebaceous hyperplasia responds well to precision removal and resurfacing, though the units producing them remain, so recurrence over years is normal and stated in advance.
