Why does filling the fold directly often look wrong?

Because the fold is a boundary between mobile and fixed tissue, and the side that sags is already carrying too much weight. Injecting volume into the groove itself adds more to that side — the crease looks flatter at rest and heavier in motion, and over repeated sessions produces the thickened, slightly simian look around the mouth that people recognise instantly. The mechanically sound approach is to reduce the load: restore midface support so tissue stops resting on the line, reposition it if it has genuinely fallen, and only then consider a small refinement within the groove.
