All five come up most weeks, and the third one usually has a diagnostic explanation. The sections below take them in order.

The clench test

Place your fingers on the widest part of your jaw, just below and in front of the ear, and clench your teeth firmly. If a substantial muscle bulges under your fingers, masseter hypertrophy is contributing and jaw botox will change your face. If very little moves and the width is there regardless, the width is skeletal — the mandible angle itself — and botox will not alter it.

What botox actually does to the masseter

Blocks the nerve signal, so the muscle stops contracting with full force and gradually loses bulk through disuse. That atrophy takes four to eight weeks, which is why the visible change arrives considerably later than with facial botox. The muscle is not destroyed and returns to its previous size over four to six months unless treatment is repeated.

What surgery does that botox cannot

Alters the bone. Mandible angle reduction removes part of the jaw angle itself, which is the only intervention that changes a skeletal wide jaw. It is a maxillofacial procedure with general anaesthesia, weeks of recovery and permanent results. Where that is genuinely what a patient needs, saying so at consultation is more useful than three rounds of botox that were never going to reach it.

Why did my jaw botox do nothing?

Three possible reasons, in order of likelihood. The width was skeletal rather than muscular, which the clench test identifies. The dose was too low for the muscle bulk — masseters vary considerably and a heavy one needs more than a slim one. Or it was judged too early, since the change arrives at four to eight weeks rather than two — see how long results last.

Why is this the most commonly misdirected treatment?

Because jaw botox is heavily marketed and the concern it treats is invisible from outside. A square jaw looks the same in a photograph whether the width comes from muscle or bone, so patients search for the treatment they have read about and clinics take the booking. The result is a treatment that performed correctly on tissue that was not producing the appearance — and a patient who reasonably concludes that botox is overrated. The clench test costs nothing and takes five seconds, which makes skipping it difficult to justify.

What does a good outcome actually look like?

A softened lower face rather than a narrow one. Masseter reduction changes the angle from square toward oval, which reads as a refined jaw rather than a different face — and on a male patient the goal shifts further, toward reduced bulk with the jaw still clearly defined. What produces an odd result is over-reduction, particularly in lean faces where the muscle was contributing structure the face needed. Conservative first dosing with a review at four to eight weeks is how that is avoided, and adding units later is straightforward — see skin treatments for men.

What about grinding and jaw pain?

That is the functional indication, and it responds well independently of any cosmetic effect. Nocturnal bruxism produces masseter hypertrophy, morning jaw ache, headaches and tooth wear — and relaxing the muscle addresses all of those. Patients who come for the pain and gain a narrower face frequently value the first more, which is worth knowing if you are hesitating about the cosmetic aspect.

Can both be present?

Frequently, and it changes the recommendation rather than ruling out treatment. A jaw that is both skeletally broad and muscularly bulky gets a partial result from botox — softened but still wide. Being told to expect partial improvement is different from being told it will not work, and many patients accept partial change over surgery once the trade is stated plainly.

What else affects how wide a jaw looks?

Two things that get overlooked. A recessed chin shortens the lower face and makes the jaw read as wider by proportion — chin filler often does more for a square appearance than the masseter does. And submental fullness blurs the jaw line, which reads as heaviness rather than width — see double chin. Treating the masseter alone in either case leaves most of the appearance in place.

What are the side effects of masseter botox?

Chewing fatigue with hard or chewy food for two to three weeks on a first treatment, which settles as the muscle adapts. Occasional temporary asymmetry in smiling if the dose spreads, correctable at the two-week review. Long-term repeated treatment can produce some hollowing at the cheek in very lean faces, which is a reason for conservative dosing rather than for avoiding it.

How should the decision be made?

By examination rather than by preference. The clench test, an assessment of chin projection, and a look at the submental area between them explain almost every wide-jaw appearance.
