By insisting the grading comes first and that every component is individually priced — which is exactly how this is structured. If descent scores low, no threads appear in the plan. If volume is intact, no filler is added. Roughly a third of patients who arrive asking about a lifting programme leave with a single component instead, because the grading showed one finding dominated. Is a combination programme better than one strong device?

Only when the grading shows more than one problem, which is genuinely common but not universal. A face whose complaint is purely laxity does better with a well-chosen energy platform than with a programme, and adding threads to it would spend money against a finding that scored low. Where combination plans earn their place is the very common picture of moderate descent plus moderate laxity plus some volume loss, where treating any one alone leaves the patient partly satisfied and unsure why.
