More often created than born, in my experience — and that is good news rather than an accusation. Genuinely innate sensitivity exists, particularly alongside atopic tendency, but the large majority of patients who tell me everything stings are describing acquired barrier damage: months of acids, retinoids, cleansing brushes, home devices and frequent treatments, each individually reasonable and collectively too much. The reason it matters is that acquired damage repairs. At our Busan clinic the first prescription is usually a list of things to stop, and four weeks later most patients have skin that behaves like it used to.

Usually yes, and that is the point of the repair phase rather than a permanent restriction. Once barrier function returns — typically four to eight weeks of simplified care — actives are reintroduced one at a time with a week between each, so that any reaction can be attributed to something specific rather than to a whole routine. Lasers and energy treatments follow later, at conservative settings, and many patients who arrived unable to tolerate a moisturiser complete pigment or texture courses comfortably a few months later at our Busan clinic.

Why do you want me to stop using products first?

Because continued damage makes everything else pointless. If acids, retinoids and devices are stripping the barrier faster than treatment repairs it, we are simply paying to run in place — and the honest first move is subtraction, which costs nothing. It is also diagnostic: skin that recovers within four weeks of simplification was suffering from over-treatment, while skin that does not is telling us there is a condition underneath.
