What is Under-Eye Filler, and what does it do?
Under-eye filler
treats the tear trough, the hollow that runs from the inner corner of the eye down and outward, which casts a shadow that reads as dark circles and tiredness. When the shadow is a hollow, a small volume of thin hyaluronic acid placed deep on the bone by cannula fills the groove, the shadow goes, and the face looks rested. When the shadow is pigment in the skin, or a bag of fat above the hollow, filler does not help and can make things worse, which is why this is the one area where the consultation most often ends with "not filler".

Questions, answered properly
Will under-eye filler remove my dark circles?
If the circle is a shadow cast by a hollow, yes, usually completely. If it is pigment in the skin, no, and we will not inject it. Most people have some of both; the doctor at Boom Dermatology tells you the split at the consultation and treats the hollow with filler and the pigment with toning, in that order.
How do I know whether my dark circles are a hollow or pigment?
Press gently on your cheek just below the circle and push upward. If the shadow fills and softens, it is a hollow and filler will work. If the colour stays exactly where it was, it is pigment. Looking in daylight from the side also helps: a hollow has a visible groove; pigment is flat.
Why does under-eye filler go wrong so often elsewhere?
Three reasons: gel placed too superficially, where it shows as a blue-grey line (the Tyndall effect); too much gel, which holds water and puffs; and filler under a fat bag, which lifts the bag. At our Seomyeon skin clinic the gel goes deep on the bone by cannula, in half to one cc per side, and bags are declined. Anything that looks wrong is dissolved with hyaluronidase within a day.
Will it make me look puffy?
Not at conservative volumes placed deep. Puffiness comes from over-filling or from filler in patients with allergies or fluid retention, which is why we ask. If you have ever had a puffy result elsewhere, say so; it changes the plan.
What if I am not a candidate?
